Abstract

В обзорной статье обсуждаются результаты недавно опубликованных исследований, касающиеся ведения пациентов с подагрой. Недавние исследования показывают, что лечение, направленное на достижение целевого уровня сывороточной мочевой кислоты, наряду с индивидуализированным подходом, образованием пациента и совместным принятием решения по поводу лечения, приводит к отличной приверженности к уратснижающей терапии (УСТ) и предотвращает обострения подагры в долгосрочной перспективе. Долгосрочная УСТ до достиже-
ния цели предотвращает обострения подагры и улучшает качество жизни пациентов. Недавно опубликованное исследование CARES демонстрирует вероятность того, что фебуксостат увеличивает риск смерти у пациентов с существовавшими ранее сердечно-сосудистыми заболеваниями, и аллопуринол должен оставаться препаратом первого выбора среди агентов УСТ. Существует мало данных о дозировке УСТ для лечения гиперурикемии у пациентов с подагрой и хроническим заболеванием почек. Тем не менее последние исследования показывают, что доза аллопуринола может быть увеличена выше обычной дозы для пациентов с хроническим заболеванием почек без синдрома гиперчувствительности к аллопуринолу. Необходимы дополнительные более крупные ис- следования, касающиеся этого утверждения. Последние исследования показали эффективность фебуксостата, сопоставимую или превышающую эффективность аллопуринола, а также продемонстрировали возможное на-
значение его пациентам со сниженной функцией почек, аллергической реакцией или резистентностью к аллопуринолу, что значительно улучшает прогноз.
