Abstract
The article presents the experience of working of professional surgeons and endocrinologists for ways to optimize the treatment of ulcer-necrotic foot defects, complicated by phlegmons and osteomyelitis. It had been studied two groups of patients with clinical type 2 diabetes with purulent-necrotic foot defects – the primary group (59 persons) and the control group (63 persons). 122 patients with neuropathic and neuro-ischemic forms of diabetic foot (III and IV stage of the process by Wagner) were operated on in both groups. The scheme of post-operative treatment of 59 patients of the primary group contained an intravenous drip of α-lipoic acid as a drug Dialipon® at a dose of 1200 mg/day for 20 days. So optimized surgical method of treatment and the expansion of indications for use of the drug Dialipon® in the postoperative period in patients with purulent-necrotic presentations of diabetic foot helped to reduce the period of healing of postoperative wounds in the primary group of patients up to 28-30 days, whereas in the control group, the healing time were 56-60 days. The developed algorithm and the scheme of complex surgical treatment of these patients allowed to minimize the number of high amputations and increased by 40% the number of distal amputations of the foot, and thus it helped to keep the foot bearing function in 92% of cases.
