Abstract

Background. Polyendocrine metabolic ovarian syndrome (PMOS) (polycystic ovary syndrome (PCOS)) remains the
most prevalent endocrinopathy among women of reproductive age (8–13%), characterized by a complex interplay of reproductive,
metabolic, and psychological disturbances. Hyperandrogenism and insulin resistance are the pivotal pathophysiological links,
necessitating the search for effective metabolic correctors.
Objective. To evaluate the therapeutic potential of levocarnitine (the drug Licartin) in correcting hormonal and metabolic
disorders in patients with PCOS based on an analysis of global data and original clinical observations.
Methods. A systematic analysis of current literature and meta-analysis results was performed. Furthermore, three clinical
cases were described involving the administration of levocarnitine at a dosage of 2000 mg (intravenous and oral) in PCOS patients
of various age groups with comorbid conditions (Type 2 Diabetes Mellitus, adenomyosis, and pubertal hyperandrogenism).
Results. It was established that levocarnitine acts as a critical modulator of fatty acid beta-oxidation, contributing to a reduction
in the HOMA-IR index and fasting insulin levels. According to meta-analysis data, adjuvant therapy with levocarnitine increases
the spontaneous ovulation rate by 3.4 times and improves endometrial thickness. Original clinical experience confirmed that the
administration of Licartin (2000 mg) for 20-30 days ensures a 12-15% reduction in free testosterone levels, a reduction in body
weight (primarily visceral fat), and the restoration of menstrual cycle regularity. Additionally, improvements in dermatological
status and cognitive functions were noted.
Conclusions. The integration of levocarnitine (Licartin, 2000 mg) into PCOS treatment protocols achieves a synergistic effect
in normalizing the hormonal background and metabolic profile. The use of both intravenous and oral forms provides a rapid
clinical response, making it an effective tool for intensive initial therapy and preconception care.
