Abstract
The 2025 Standards of Care in Diabetes by the American Diabetes Association (ADA) present major updates that emphasize a multidimensional, patient-centered approach. Continuous glucose monitoring (CGM) is now recommended regardless of insulin use, expanding its role in type 2 diabetes management. Greater priority is given to GLP-1 receptor agonists and dual GIP/GLP-1 agonists, not only for glycemic and weight control but also for cardiovascular, renal, and hepatic protection. Pioglitazone remains an important option for type 2 diabetes and its complications. Resmetirom, as a new drug, is highlighted for metabolic dysfunction–associated steatotic liver disease (MASLD). Prevention strategies include early autoantibody screening and immunotherapy (teplizumab) for type 1 diabetes, and pioglitazone alongside metformin for type 2 diabetes. Special guidance addresses pregnancy, older adults, and high hypoglycemia risk. Lifestyle recommendations stress plant-based proteins, fiber, hydration, physical activity, and reduced intake of ultra-processed foods. The document also underscores diabetes self-management education (DSMES) and non-stigmatizing communication. Overall, ADA 2025 shifts from glucose-centric management toward comprehensive care to improve outcomes, prevent complications, and enhance quality of life for people with diabetes.
